Abstract
At present, the situation of Nigeria health care system vividly shows that health records are not
fully maximized to provide a strategic means of tracking modifiable diseases at the community level. The purpose of this study is to explore and reveal how health records for the patients are
generated at the primary health care level and how health personnel access health records and
whether health records are managed according to policy stipulated by National Health Management Information System (NHMIS) as contained in the national health policy as well as determine how health care providers utilize health records for health planning and patient care. The study used a qualitative approach in which a focus group guide and an interview guide were used to investigate the dynamics of health records generation, access and utilization of health records, among twenty senior health personnel drawn from all the units at the community health center in Ikenne Local Government, Nigeria. Findings in the study showed that health records are generated from basic clinical, diagnostic and nursing activities at the time of first contact with patients and records are not systematically sent to the PHC headquarters. Access to health records of patients is limited to within the health centre and little or no network of information or compliance to health information reporting protocol exist. The study recommends a reformation of the health management information system to strengthen collation of reliable health records.
