Factors influencing adherence to TB treatment among male clients attending DOTS clinic inAbeokuta, Ogun State, Nigeria

Abstract

Objectives: Tuberculosis (TB) is a deadly infection whose burden of disease is still poorly understood and managed in resource-limited settings. Due to its vast public health implications, it is one of the three communicable diseases specifically mentioned in the Millennium Development Goals. This study investigated factors affecting Adherence to TB treatment.

Materials and Methods: 254 men attending 4 DOTS centres in Abeokuta South LGA, Ogun state were recruited into the study using the purposive sampling method. The descriptive, cross-sectional study design was adopted and data collected via the administration of a semi-structured questionnaire. Data was analyzed using simple frequencies and Chi-square tests (p?5%) and presented in tables.

Results: The result revealed that 139 (54.7%) did not know that coughing out blood and night sweat are classic symptoms of active tuberculosis. 164 (64.6%) had no idea that chest pain and weight loss are symptoms of tuberculosis. However, majority of respondents knew TB is airborne (86.6%), affects the lungs (87.4%), its transmission increases in people with immune system disorders like AIDS (60.2%) and that Directly Observed Treatment Short-Course (DOTS) is used to manage patients (53.5%). Respondents correctly perceived the expectoration of sputum (56.3%) and coughing/sneezing (85.4%) as routes of tuberculosis transmission. About a third (29.9%) were non-compliant with drug regimens. Factors promoting adherence include follow-up test and the quality of services rendered.

Conclusion: The study showed that there was a dissonance between the respondents' knowledge and adherence as good knowledge but did not translate into adherence (practice) for all. Recommendation to DOTS centres and Government includes intensification of health education to individuals, communities and all TB patients, particularly at the beginning of treatment and the strengthening of health networks for adequate follow up of patients who interrupt TB treatment.
